Step Up to Better Joints

"Are you kidding?" A patient named Bob raised his voice during a visit earlier today at my office, "You are sending me to exercise even though I have pain when I move?"

Bob is a sales executive who travels all over the country for his company. Like most of my male patients, he seldom sees me unless something really bothers him. Late last year, his wife had to insist he have the physical that was supposed to have happened 2 years ago when Bob turned 50.

But today he came to see me because of right knee pain that started a few months ago, and is not letting up.

There was no recent injury to the knee, which Bob can remember. Before he turned 40, Bob used to play hockey. There could have been a few falls (and fights) a long time ago, but nothing that really crippled him, even temporarily. When he got married at age 29, Bob was 5' 8", 150 lbs, and in top physical shape. But now, over 20 years later, with decades of a busy work and travel schedule, he is at least 10 lbs overweight.

After I listened to his story and examined him, I concluded that he has run-of-the-mill arthritis. Among the recommendations I made was physical therapy.

That's when he thought I was kidding.

No, I was not kidding. Exercise DOES improve knee arthritis.

In 2012, The US government commissioned a large-scale review on the effect of physical therapy on knee arthritis. The scientists reached the following conclusions based on 422 high quality studies:

· Aerobic exercises can reduce pain and improve knee function

· Strengthening exercises can reduce pain and improve knee function

· Balance-increasing exercises can reduce knee pain

· Adverse events from physical therapy are not common

A separate large-scale study in 2011 reviewed another 28 high-quality studies. This time it focused on the effect of physical activity on knee structures. These scientists also found that physical activity can help keep cartilage in top shape. In their words, physical activity is "beneficial, rather than detrimental to joint health."

Of course, Bob is not going to start sprinting the first time he sees his physical therapist. The therapist will help him work around his pain, not through the pain. They will work together to lessen stiffness and improve range of motion. They will build stronger supporting tissues for his knee joint. If all goes well, with time his joint will be stronger in both structure and function. Most likely his pain will be much improved or go away completely.

If you want to use physical activity to strengthen your knee joints, whether for pain or prevention, use the same cautious approach.

First, like Bob, if you have pain that does not go away, check with your doctor. In addition to run-of-the-mill arthritis, there can be other conditions causing your pain that require different treatments.

Along with exercises that strengthen your knee or other joints, work with your doctor or therapist to create a physical activity plan for your whole body. You will find that your health and fitness levels will go farther with a whole body approach.

But, do you know that being overweight also makes you more likely to have arthritis in your hand joints, even though your hands do not have to support much of your body weight? Think of all the aging people you know who could reduce or eliminate joint pain just by taking off the extra pounds.

Try Low-Impact Exercises

Low-impact exercises are not necessarily low-intensity exercises. You can add speed to any of them to kick up the intensity a notch (or several notches). If you find walking not physically challenging enough, try walking the stairs.

Gentle yoga practices such as Iyengar yoga have been shown to ease arthritis. If you are new to yoga, try to work with a qualified instructor first. It is important to remember to avoid poses and movements that give you pain.

Mix Up Different Physical Activities

As found by the large review study mentioned earlier, low-impact aerobic exercise, tai chi, and yoga all benefit people with arthritis. So, mix up the cardio, strength training, and balance exercises to make a varied and integrated exercise program. Variety also makes exercise enjoyable. Listen to Your Body

This principle applies when you are doing physical therapy to ease joint pain. It also applies if you are just exercising to reap the benefits of having strong and flexible joints.

So stop if it hurts. Do not overdo any exercises, especially if you are new to it.

Starting low with a low-impact physical activity and slowly increasing the intensity is always a good practice. Warm up well before your exercises. This also applies to stretches.

If you want to treat or prevent arthritis pain, work with a trainer or instructor if possible. And, if you have any chronic illness especially of the heart or lungs, always check with your doctor first.

Why Is Losing Weight Hard or Should We Focus On Losing Fat?

I must say that I do have a problem with listening to skinny people who never had no more than 15 pounds to lose tell me about obese. I believe the best teacher is one that has had the experience or extensive training and their work is proven to be effective.

I wasn't always obese. I was once in my younger days very athletic. I played football, wrestled, lifted weights and trained in Boxing, Kung-Fu and later on Tae Kwon Do (Korean karate) and as of today hold a 5th degree black belt in Shotokan Japanese Karate.

Years ago I would get upset when the Doctor asked how I gained the weight. This was an expert obesity doctor asking me the question. That's why I chose to see him searching for an answer.

I was scrambling my brains trying to return an answer. The best I could say is - maybe it's the sweets and bread. He replied that it's probably because I most likely eat 4,500 calories daily! WOW! I really could not accept that! I thought he was crazy diagnosing me with an eating disorder.

My daily meals were the average. I followed the Food Pyramid which the Food & Drug Administration approved except for a few honey buns over the weekend.

I was about 25 years old and barely knew about counting calories and carbs. All of that was a foreign language to me at that time.

20 years later and plenty of unwanted fat, I've learned quite a lot. Why? Because it's my duty to correct the problem and the only way to correct the problem is to be educated, build legitimate tools and resources to win the battle of obese!

There are so many pills, potions and magic powder over the counter to try for weight loss. The question is should we focus on weight loss or fat loss?

YES! There is a difference.

Our body is created with a survival mechanism to store FAT when it thinks we are starving. This is the reason why diets don't work. Most of them restrict your caloric intake by lowering the amount of food to eat.

This method of losing weight throws a rod in your hormones which causes hormonal imbalance. Your hormones don't see food and calories like the naked eye. Therefore it should make sense to say the less food and calories you receive the more weight you should lose. WRONG! WRONG! WRONG!

THE TRUTH! The less food and calories the more fat your body will store to survive! You just turned on your survival mechanism to STORE FAT!

We have six fat burning hormones and three fat making (storing) hormones. The one we all should know about that stores fat is "Insulin" and it's a serious problem when you eat the wrong type of foods, it's not so much of the quantity that matters!

If eating small amounts of food were the problem why do some smaller people eat more than big people (and never gain weight) and why some big people eat less than small people? And have the hardest time losing weight!

OH! Their metabolism is good, right? Okay!

Question: What controls the metabolism? Answer: Hormones!

Hormones are chemical messages produced by glands. They send messages such as I'm sick in the liver therefore fluid could be retained in my pot belly, I'm sick in the adrenaline therefore too much cortisol is causing my outrageous belly fat! 

Yet so many scientist and big pharmaceutical create magic pills to lose weight. And the doctors drill us on taking the synthetic poison that killed millions a few years ago by blowing heart valves, and the manufacturer countered by saying the people already had the heart problem prior to taking their (poison) medicine.

The type of foods we should digest and control the hormone "insulin" is called "Low Glycemic" and we should also eat meat in the fish, turkey and chicken family because it's less fat, sugar and sodium. Food that are high in sugar, fat and sodium will trigger the insulin by spiking the levels up and dropping very fast and send a false signal of hunger.

When this happens you lose the ability to recognize satiety. You never feel it and over consume the appropriate quantity of food. The reality is - you consume an extraordinary amount of high glycemic foods. 

The results - Obese - Morbid Obese - Super Morbid Obese!

What we are taught is to visit our primary physician and then they ask you - how did you get like that? WOW! Doc that is why I'm here sitting before you!! I thought maybe you could use your expertise and provide the answer oppose of me scrambling my brains hoping to produce an answer.

Next the physician warns you about the high risk you are in because of the excessive weight gain;

It was far safer to stay at home. To try and break this cycle I decided to attempt to talk to him as his vehicle pulled in the driveway. I used a strategy of counting in my head whilst considering a beach scene which I found relaxing.

It created a calm place for me to being a conversation. Initially I found this thinking technique difficult but over time it worked for me.

I began by exchanging a hello and some polite basic conversation. More than a number of times my anxiousness levels had been decreased and I was in a position to move onto the following stage before finally having that coffee.

Speaking Your Mind

The dance pole workout is very popular these days because let's face it going to the gym can become a mundane task that takes a lot of motivation. Working a full-time job only to reward yourself by subjecting your body to physical torment is one reason America is the fattest country on earth.

Thirdly and lastly the legs receive tremendous benefit from all of this as well. The legs are often used to grip the pole and even just the dance moves that are included promote muscle tone.

The biggest problem that this author sees, in the US today, when it comes to what foods are optimal for weight loss and health, is the confusion over the subject. One expert says one thing, while another expert completely contradicts the first one and says something totally different.

I feel that a little bit of thinking and common sense goes a long way towards solving the dilemma of what foods are beneficial for weight loss and our declining health statistics.

If we lose the weight, but in the process, ruin our health, it is all for nothing as we will soon gain it all back plus some and our health will be closer to the brink of serious, possibly debilitating disease. This is the situation I was faced with about 2 years ago, and it caused me to get really serious about my health. I did a lot of research on the subject.

What I found when I started looking at the data and the conclusions I reached were really common sense, and I think if you will do the same, you will most likely come to the same commonsensical conclusion.

When you look at the graphs of escalating levels of obesity, diabetes, heart disease, celiac disease, cancer, autism, ADD, IBS and many other "modern" diseases that are far too numerous to mention here with the increasing consumption of refined foods, High Fructose Corn Syrup, sugar, genetically modified crops, feedlot animal products, dairy where the cattle are injected with growth hormones for increased production, meat that is treated with antibiotics to offset the diseases caused by unnatural grain diets and vegetables that are treated with pesticides the graphs are almost identical in their rise from 100 years ago to today.

What is going on here?

Medical Science has progressed by millennia with the advent of super computers that can do in minutes now, what used to take years. So why, with all of this "progress" are we regressing when it comes to our health as a whole?

Why is obesity and diabetes increasing, why is heart disease progressing, why are cancers more commonplace? Why are ADD, Celiac Disease, autism and countless other maladies going through the roof, when only 50 years ago many were almost unheard of or at least extremely rare?

What is different? When you do a little research, it is overwhelmingly obvious. Our food is different. What we eat on a regular basis as normal American food has undergone monumental changes for the worse in the past 50 years.

We are a society that values and embraces progress. I propose to you that we are regressing, not progressing as far as weight gain and health matters are concerned.

So, what do you do about it and how does this concern weight loss?

The answer is, you rewind the clock about two hundred years when it comes to making food choices. It really is as simple as that. The wheat we eat today is far different from the wheat our grandparents ate and that applies to soy products, corn products and many others. It also applies to mass production feedlot animal and their products, refined oils that are touted as "good for you" because they are easy and cheap to produce, and vegetables that have pesticides sprayed on them to increase production.

Many, extensively researched books could be written concerning this subject, but that is beyond the scope of this article and my only goal here is to bring to your attention, what I have found to be the worst offenders of abnormal weight gain and worse, diseases caused by diet in the western world.

When I changed my diet to line up with this, the weight fell off and all of my biomarkers of health improved drastically. It is up to you to further research what I am proposing as a fit and healthy lifestyle. A lifestyle change that has done wonders for me, my family and many friends.
